An estimated 30% of Americans with Major Depressive Disorder find no relief from traditional treatments, leaving them with persistent and often worsening symptoms. But a controversial diet, the ketogenic diet, has shown promising results in a recent study, suggesting it could be a game-changer.

The ketogenic diet, often referred to as "keto," is a high-fat, moderate-protein, low-carb eating plan. It's designed to shift your body's metabolism, encouraging it to burn fat for energy instead of sugar. This metabolic shift produces ketones, an alternative fuel source for the brain and muscles, which scientists believe could benefit brain health.

Previous research has hinted at the potential benefits of ketones for the brain. They may provide a more stable energy supply, regulate mood-related neurotransmitters like serotonin, reduce inflammation, and even improve gut health - all factors that are linked to depression.

In this study, researchers recruited 88 adults with treatment-resistant depression, all scoring over 15 on the PHQ-9 depression screening tool. They were divided into two groups: one following a strict keto diet, with three prepared meals and snacks daily, and the other following a "phytochemical" diet, incorporating a variety of fruits and vegetables and swapping animal fats for plant oils.

After six weeks, both groups showed improvements, with depression scores dropping significantly. The keto group saw an average drop of 10.5 points, while the phytochemical group saw an average drop of 8.3 points.

However, maintaining the keto diet proved challenging. Once the prepared meals and weekly support ended, only 9% of participants continued with the diet.

Dr. Raphael Braga, a physician at Northwell Health's Zucker Hillside Hospital, who was not involved in the study, commented, "The main takeaway is that keto might have some value, but it's a very restrictive measure." He noted that metabolism plays a role in the potential benefits of keto for severe depression, but it's not the only factor.

Braga suggested that the support and structure provided to participants throughout the study could have been a significant factor in their improvement. The prepared meals, regular check-ins, progress tracking, and counseling sessions all contributed to a supportive environment, which is crucial for managing severe depression.

While keto is often associated with weight loss and fat burning, it's important to consider its potential downsides. Some studies have linked keto to poorer diet quality and increased LDL cholesterol levels. A 12-week study found participants had higher cholesterol, increased levels of apolipoprotein B (which contributes to artery plaque), and lower levels of Bifidobacteria, a beneficial gut bacteria.

Braga emphasized that other lifestyle changes, such as weight management, exercise, and routine management, can also make a significant difference for those with drug-resistant depression. Taking care of one's health, losing weight, or maintaining a healthy weight can be powerful tools in managing depression.

So, while the ketogenic diet might offer a unique approach to managing drug-resistant depression, it's not a one-size-fits-all solution. The study's findings highlight the importance of a holistic approach, combining dietary changes with supportive measures and lifestyle adjustments.
